At Moshiri Orthodontics, MARPE treatment is led by Dr. Charlie Boehm, who places and treats our MARPE cases with a slow, controlled approach designed to support skeletal expansion, airway-focused treatment, patient comfort, and long-term stability.
MARPE is a precision-controlled skeletal expander supported by small mini-implants called TADs. Unlike traditional expanders that rely mostly on the teeth, MARPE helps guide expansion at the skeletal level, creating space in the upper jaw and supporting improved bite alignment.

What to Expect
During MARPE treatment, patients may notice mild pressure across the palate, nose, or cheeks, temporary tightness after turns, speech changes, increased saliva, and a space forming between the front teeth.
That space is expected and is often a positive sign that expansion is occurring. It will be closed later in treatment.
Our Approach
We do not rush MARPE expansion. Our philosophy is to use slow, controlled activation to support comfort, skeletal development, airway goals, and stability rather than fast or forceful expansion.
Treatment Timeline
The active expansion phase typically lasts 6–8 months, depending on the patient. After expansion is complete, the appliance remains in place while the bone stabilizes. The MARPE may stay seated for up to 12 months.

Some MARPE patients may also wear aligners during treatment. Your aligner schedule and activation instructions will be customized by our team, and we will walk you through exactly when to turn the appliance and when to change aligners.
Daily Care
Patients should brush carefully around the appliance, bands, and TADs. A proxy brush and water flosser can help keep the appliance clean. Sticky, chewy, and hard foods should be avoided to protect the appliance.
